The St. Elizabeth Healthcare Ambulatory Clinical Pharmacist is responsible for direct patient care via pharmacist/practitioner collaborative care agreements for disease states in dermatology, endocrinology, hepatitis C, inflammatory bowel disease, HIV, HIV PrEP, rheumatology, hyperlipidemia, osteoporosis, pulmonology, neurology, and more. Patients are seen by the pharmacist to help optimize specialty medication therapies, improving patient outcomes, safety, and cost utilization. The ambulatory clinical pharmacist may also have patient care responsibilities in anticoagulation and erythropoietin management.
