DescriptionThe Population Health Pharmacy Program supports Population Health initiatives to increase the quality of care, while maintaining cost-effective outcomes through disease and medication management programs. The Pharmacy Technician is an integral member of the Faculty Practice Group’s Office of Population Health and Accountable Care (OPHAC) team and will work closely with the Pharmacist to execute strategies for the Population Health Pharmacy Programs as it relates to value-based care incentive programs.
