1. Solid Organ Transplant Clinical Pharmacy Specialist
The solid organ transplant pharmacist collaborates with pharmacists, physicians, nurses, and other providers to ensure safe, and effective use of medications in the abdominal and/or cardiothoracic transplant service lines, serving as an expert of transplant pharmacotherapy. This clinical pharmacy specialist requires application of training to actively participate on interdisciplinary rounds, provide transplant education for patients and healthcare professionals, develop transplant protocols and guidelines, and implement quality improvement and research projects. This position will have the opportunity to practice in a variety of settings including transplant ICU and ambulatory care transplant patients. The pharmacist will also be responsible for collaborating with other pharmacists to support departmental projects and initiatives and serving as a preceptor for students, PGY1 residents, and PGY2 residents.
2. Hem/Onc Clinical Specialist
The Cancer Center at Beth Israel Deaconess Medical Center delivers advanced cancer care with personalized treatment, clinical excellence and compassionate care. The Pharmacy Department is recruiting for a Clinical Specialist to support inpatient and outpatient hematology/oncology and cellular therapy services. The Clinical Specialist will be expected to participate in multidisciplinary patient care rounds, collaborate with providers and nursing staff, clinically review and assess chemotherapy treatment plans, and dispense oncolytic medications. This position will support patient and provider education, guideline/policy development, and pharmacy student precepting. Required Qualifications: PGY 1 + 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area. Preferred Qualifications: BPS Certification. Work Shift: Rotating.
3. ICU/ED Float (30 hour)
The float ICU/ED pharmacist collaborates with pharmacists, physicians, nurses, and other providers to ensure safe, and effective use of medications in adult intensive care units and the emergency department. This clinical pharmacy specialist will work a rotating schedule depending on the needs of the team. The pharmacist will be responsible for approving all medication orders and responding to drug therapy questions promptly. The pharmacist will respond to medical emergencies (e.g., adult Code Blue, Stroke Code, etc.). The pharmacist will also be responsible for educating pharmacists, nurses, advanced practice providers, resident physicians, and attendings about medications and treatment protocols used to care for ED and critically ill patients. The pharmacist may also serve as a preceptor for students, PGY1 residents, and PGY2 residents.
General Clinical Pharmacist Specialist Job Description:
1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures.
2. Reviews and verifies medication orders with consideration, including but not limited to the following patient-specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.
3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.
4. Provides drug information to physicians, nurses and other health care professionals.
5. Checks and verifies sterile and non-sterile medications.
6. Supervises the activities of students, technicians, and residents within the pharmacist area of practice.
7. Promotes a culture of patient safety by submitting medication-related events and near misses.
8. Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.
9. Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department. Precepts pharmacy students, resident rotations, and resident-led quality improvement projects.
10. May serve as Residency Program Coordinator or Residency Program Director
11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.
12. Supports a specific core area of practice through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff.
13. Updates clinical guidelines and presents hospital wide educational initiatives.
14. Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes. 15. Supports IRB-approved research activities, medication use evaluations, or cost-savings initiatives.
