The Hospital of the University of Pennsylvania is establishing the Thrombosis and Hemostasis Pharmacy Residency Program, a comprehensive 52-week full-time commitment. The aim of this second-year pharmacy residency is to provide advanced training and clinical expertise to licensed pharmacists who have completed a PGY1 residency, in the specialized field of anticoagulation and hemostasis. The program focuses on developing skills and knowledge for various clinical roles, such as inpatient and outpatient anticoagulation management, consultation services, and active participation in anticoagulation stewardship programs.

Residents play a crucial role in the multidisciplinary team, ensuring the safe and effective use of anticoagulants, educating providers, patients, and caregivers, participating in selected meetings, and facilitating transitions between care settings. Longitudinal experiences encompass pharmacy operations, medication adherence assessments, clinical research, and professional development, including opportunities for presentations, teaching, mentoring, and writing.

The program is customized for each resident based on their goals, interests, and previous experience. Required rotations last one month, covering areas such as Anticoagulation Inpatient Management team, Benign Hematology Consult, Thrombophilia Clinic,  Surgical Intensive Care Units, Pharmacogenomics and Pediatric Thrombosis program. Additionally, residents undergo a three-week clinical laboratory tutorial on hematologic interpretation, instrumentation in the hematology laboratory, and interpretation of coagulation studies.

Furthermore, the Outpatient Anticoagulation Management Center (AMC) functions as a multidisciplinary unit, overseeing the ambulatory anticoagulation requirements of more than 1400 patients annually. This collaborative team is comprised of nurse practitioners, pharmacists, clinicians, practicing nurses, and administrative staff who work collectively to deliver comprehensive anticoagulation care.

The anticoagulation pharmacy group actively engages in clinical research, collaborating on multiple projects with PGY1 and PGY2 residents each year. Residents also participate in ongoing prospective trials through enrollment and screening.
