Program Overview The Thrombosis and Hemostasis Management Residency Program (PGY2) is a 12-month program located at the M Health Fairview University of Minnesota Medical Center in Minneapolis, Minnesota. The residency is currently in pre-candidate status with ASHP with plans to pursue accreditation after graduation of the first resident. This program prepares the resident to become an advanced practice pharmacist through a broad range of learning experiences in areas of hemostasis and anticoagulation stewardship. At the M Health Fairview University of Minnesota Medical Center, we balance the latest in technology and treatments with patient-centered care. We provide care for a wide range of specialties including, medicine, critical care, transplant-both solid organ and stem cell, and pediatrics. As the core teaching hospital of the University of Minnesota Medical School and College of Pharmacy, our hospital is among the most respected in the nation, providing superior health care results. Additionally, the University of Minnesota Medical Center is one of three recipients of the Anticoagulation Forum TRAIN Grant for PGY2 Thrombosis and Hemostasis Residents in 2025-2026 and 2026-2027. As part of the TRAIN Grant, the resident and residency program director are required to participate in bi-weekly topic discussions with the other TRAIN Grant sites.
PGY2 Program Purpose: The purpose of this PGY2 residency program is to build upon Doctor of Pharmacy (PharmD) education and PGY1 pharmacy residency training to develop pharmacist practitioners with knowledge, skills, and abilities as defined in the educational competency areas, goals, and objectives for advanced practice areas. Residents who successfully complete PGY2 residency programs are prepared for advanced patient care or other specialized positions, and board certification in the advanced practice area, if available.

Candidate Qualifications: • PharmD Degree • Minnesota licensure eligible • PGY1 pharmacy practice residency program completion • Graduate of an ACPE program or FPGEC certificate
