The Ambulatory Clinical Pharmacy Specialist provides direct patient care services for patients with chronic illnesses, promote health and wellness, and participates in innovative patient visits including transitions of care, patient education classes and shared visits. 

Essential Duties & Functions: 

• Develops and maintains a clinical practice with a patient care service with multidisciplinary team to optimize pharmacotherapy for patient care
• Provides optimal selection of pharmacologic agents in designing regimen to maximize therapeutic effect and minimize cost and toxicity
• Assesses medication adherence during the process of medication counseling and works toward removing the potential barriers for medication access 
• Provides other health professionals with drug and clinical information 
• Provides education and counseling about the drug therapy provided to patients 
• Performs physical assessments and triaging patients as needed
• Orders labs and other tests to help determine efficacy and toxicity of medication therapy using approved delegating procedures
• Participates in clinical guideline development, collaborative practice agreements in chronic disease state management and drug utilization reviews
• Provides clinical education and training to students, pharmacists, residents and medical providers 
• Participates in quality assurance programs in the Department of Pharmacy
• Supports and performs operational components in the pharmacy
• Serves as preceptor to students of affiliated schools of pharmacy and pharmacy residents (when available) 

Performs other duties as assigned 

Requirements:

• Graduate of an accredited college/university of Pharmacy (PharmD or B.S. with equivalent experience)
• Completion of an ASHP accredited PGY-1 Pharmacy Residency Program

Preferred:

• 2+ years of experience in Ambulatory Clinical Pharmacy setting 
• Completion of an ASHP accredited PGY-2 Pharmacy Residency Program for Specialty Area
• Board Certified Pharmacotherapy Specialist (BCPS) or Board Certified Ambulatory Care Pharmacists (BCACP)
