ProMedica utilizes the Pediatric Pharmacist as a liaison for safe and effective medication therapy for the pediatric populations served. The Pediatric Pharmacist works collaboratively with members of various pediatric teams and general medical staff members through daily medication review activities, rounding, and reporting with additional key responsibilities listed below.  The Pediatric Pharmacist serves as an expert for the department for related subject matter education and practice change, a preceptor for multiple levels of pharmacy learners, and a swift link for communication to our pediatric teams.

ACCOUNTABILITIES /COMPETENCIES

1. Serve as content expert in pediatric practice for pediatric providers, pharmacists, nursing, and other members of the patient care team.
2. Provide support for pediatric specific quality reviews and endeavors.
3. Participate in antimicrobial stewardship rounding with pediatric infectious disease practitioners PRN.
4. Review pediatric inpatients for assigned area for opportunities to improve patient care, optimize patient or caregiver education, and facilitate medication alternative delivery routes PRN.
5. Round with the prescriber teams in assigned area.
6. Monitor, evaluate, and report pediatric specific adverse reactions and medication errors.
7. Assist in developing and maintaining pediatric-related local guidelines, antimicrobial use policies, order sets, and clinical pathways.
8. Review new formulary agents for distinct pediatric build needs.
9. Develop and deploy departmental competency, training, and education.
10. Review pediatric specific EHR builds for appropriateness.
11. Provide build suggestions for practice area in smart pump libraries.
12. Regularly participate in pediatric provider meetings to facilitate communication.
