The PGY2 Solid Organ Transplant Pharmacy Residency at UCSF Medical Center is an American Society of Health System Pharmacists (ASHP) accredited residency program. Graduates of the PGY2 Solid Organ Transplant Pharmacy Residency are prepared to manage heart, lung, kidney, pancreas, and liver transplant recipients in inpatient and outpatient settings, as well as patients with viral hepatitis and opportunistic infections, and conduct clinical research. The resident will also have the opportunity to manage pediatric transplant recipients at the UCSF Benioff Children's Hospital. The resident is an integral part of the multidisciplinary team, and will interface with the span of healthcare providers managing the transplant patient across the continuum of care. Teaching is an integral component of the program, as UCSF Medical Center is affiliated with the UCSF School of Pharmacy. The resident will be involved with the teaching of students, PGY1/PGY2 residents, and other healthcare providers. The resident will also have the option to complete a teaching and research certificate program.
UCSF is home to one of the largest kidney and liver transplant programs in the nation. UCSF consistently has been one of the most active kidney transplant program in the United States, performing over 400 kidney transplants annually. The liver transplant program has a one-year survival rate of 96%, which is above the national average of 92% even when adjusted for high-risk patients, and is also one of the largest, with about 200 liver transplants performed each year. UCSF also has a large living kidney and liver donor program, and is one of few centers that perform transplants in HIV positive individuals. UCSF also has one of the most successful lung transplant programs in the nation, has a reputation for accepting challenging cases and outcomes significantly better than expected survival years following transplant. The heart transplant program is rapidly expanding and is expected to perform about 70 transplants. Adult transplant services are based at the Parnassus campus of UCSF Medical Center, and pediatric transplant services are performed at the UCSF Benioff Children’s Hospital.

The liver transplant program has been designated a center of excellence by the U.S. Department of Health and Human services. There were over 200 liver transplants (deceased and living donors) performed at UCSF last year. Since 1991, the lung transplant program has expanded year after year. UCSF has established the reputation for accepting challenging and complex cases with outcomes consistently exceeding the national average. Over 100 patients received a lung transplant at UCSF last year. The heart transplant program is rapidly expanding, and is expected to perform about 70 transplants.
Transplant pharmacists are integral members of the transplant teams in both the inpatient and outpatient setting. The transplant pharmacist actively manages the medications of transplant recipients during their hospital stay and facilitate their transition to home. Transplant pharmacists also see patients in post-transplant clinics. The transplant pharmacists also serve as preceptors for the UCSF PGY2 Solid Organ Transplant Pharmacy Residency, the PGY2 program and students with the UCSF School of Pharmacy. Transplant pharmacists also have prescriptive authority under collaborative practice agreements.
