Clinical pharmacists work collaboratively as part of the care team to ensure safe, effective, and affordable medication therapy for patients in their care. Pharmacists working in Pharmacy Primary Care Services are embedded in our primary care clinics and are tasked with caring for patients with chronic conditions and complex medication regimens.
Focus is placed on the Comprehensive Medication Management framework and providing patient education on health and lifestyle changes. Pharmacists practice under a broad collaborative practice agreement engaging with patients, providers, support staff, care management, and social work. Clinical pharmacists work with trainees in the department (eg, students, interns, residents, and new staff).
Responsibilities
Perform medication reconciliation with patients and caregivers.
Optimize medication regimens through comprehensive medication management with focus on indication, efficacy, safety, and adherence.
Provide medication education and counseling.
Precept health care students and residents.
Identify and engage in quality improvement activities.
Required
Demonstrated human relation, effective communication, and computer literacy skills.
Additional qualifications may be required depending on area the pharmacist is primarily assigned to work. Examples of additional qualifications could include basic or advanced life-support training, residency training, or certifications.
Licenses Required
Current licensure to dispense controlled substances in the State of Utah.
