The critical care clinical specialist pharmacist will manage and operate a comprehensive cardiovascular surgery pharmacy practice providing the scientific knowledge and professional judgment required to improve and promote health through assurance of the safety, efficacy, and cost-effective use of drugs.
Responsibilities
Provides comprehensive pharmaceutical care to patients to ensure safe, effective and cost-effective use of drugs on an academic cardiovascular surgery critical care service. Demonstrates independent decision-making, clinical judgment and in-depth knowledge of critical care therapeutics. Develops and maintains a clinical practice, collaborating with medical and nursing personnel in pursuing optimal drug therapy for cardiovascular surgery patients. Provides education and medication-related counseling to patients, families and caregivers about the drug therapy provided. Recommends evidence-based adjustments or modifications to the drug regimen according to clinical and toxicity endpoints, advanced support therapies utilized, and professional guidelines. Provides drug and clinical information to other healthcare professionals related to critical care practice. Provides clinical education and training to pharmacy students and residents. Participates in medication use evaluation, guideline and policy development, prepares documents for P&T evaluation/consideration (formulary, etc.) and other projects as assigned. Participates in committees, including P&T Committee, Medication Safety, critical care services, and others relevant to the care of cardiovascular surgery patients. Develops/maintains operational protocols and procedures in accordance with regulations and professional guidelines and Vandalia Health CAMC policy. Participates/leads performance improvement activities aimed to improve the safety, quality, cost effectiveness and efficiency of patient care. Demonstrates positive customer service and co-worker relations.
Doctor of Pharmacy from an ACPE accredited institution
PGY1 pharmacy residency/PGY2 pharmacy residency in critical care
