Ambulatory Pharmacy Specialist – Rheumatology Clinic [actively recruiting] The ambulatory pharmacy specialist will provide direct patient care alongside rheumatology at WVU Hospitals in Morgantown, WV. The pharmacist will participate in multidisciplinary patient visits with rheumatology providers in addition to independent visits scheduled with the pharmacist to monitor and adjust medication therapy under collaborative practice. The pharmacist will collaborate with infusion centers, specialty pharmacies, and other teams as appropriate to facilitate appropriate medication access. Other daily activities will include precepting students and residents, participating in quality improvement initiatives and committees, and providing education to multidisciplinary teams.
Ambulatory Pharmacy Specialist - General Cardiology [actively recruiting] This position will work directly with providers and patients in the WVU Heart and Vascular Institute, specifically in the Cardiology Fellow's clinic. The pharmacist will assist providers with medication-related patient care with opportunity for independent and longitudinal patient management via collaborative practice.
Ambulatory Pharmacy Specialist – Family Medicine [pending approval] The ambulatory pharmacy specialist will work directly with providers with the WVU Department of Family Medicine discharge and primary care clinic locations in Morgantown, WV. Proposed clinic responsibilities include comprehensive medication review and management, patient and provider education, and clinical recommendations for return hospital discharge visits. The pharmacist will have the opportunity to participate in multidisciplinary visits with providers and to lead pharmacist appointments via physician-directed referrals and collaborative practice.
Ambulatory Pharmacy Specialist – Neurology [pending approval] The ambulatory pharmacy specialist will provide direct patient care alongside providers in the Rockefeller Neurosciences Institute in Morgantown, WV, specifically within the memory health clinic and the comprehensive movement disorder center. The pharmacist will support the providers in these spaces through multidisciplinary patient visits with the possibility for independent scheduled visits with the pharmacist under collaborative practice and physician-directed referrals.
The minimum and preferred qualifications for all positions are as follows:
MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Bachelor of Science in Pharmacy OR Doctor of Pharmacy (Pharm.D.) from an ACPE accredited school of pharmacy.
2. Must possess current and unrestricted license to practice pharmacy in West Virginia or be license eligible. Licensure must be completed within 90 days of hire. If licensure is not obtained within 90 days of start date, associate will be notified that they will be released from their employment.
3. Obtain certification in Basic Life Support within 60 days of hire date.
PREFERRED QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Pharm.D from an ACPE accredited school of pharmacy.
EXPERIENCE:
1. Completion of a PGY-2 Residency in an applicable clinical field OR;
2. Completion of a PGY-1 Residency AND Board Certified in an applicable clinical field OR;
3. Completion of a PGY-1 Residency AND three years of relevant pharmacist experience OR;
4. Three years of relevant pharmacist experience AND Board Certified in an applicable clinical field.
