Job Description:

This position is located at Mayo Clinic in Rochester, MN. The Hematology/Oncology pharmacist provides medication management services in specialized Heme/Onc clinic areas or as part of the inpatient care team.  The pharmacist works as part of a Cancer Care team to design, implement, monitor, evaluate, and modify patient cancer therapy to ensure that medication therapy is indicated, effective, safe, and convenient.  Completes documentation for pharmacist activities.  Participates in collaborative practice and/or protocol-driven practice, as appropriate. Teaching includes multispecialty team members, as well as Pharmacy Students and Residents (PGY1 and PGY2), and precepting and mentoring Heme Onc PGY2 Residents. Participates in quality improvement and resource management initiatives.  Participates in and supports research activities of the department and the clinic areas served. We have clinical pharmacy services weekdays Monday-Friday while the clinic is open.  For inpatient, services are provided 7 am-10:30 pm, Monday-Sunday.

Job Requirements: 

• Required: Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree.
• Preferred: A PGY1 Pharmacy Residency and a PGY2 Oncology Residency or equivalent experience in the Heme/Onc practice. BCOP or BCOP eligible.
