 About the Role We are seeking a highly motivated and clinically skilled Advanced Practice Pharmacist (APh/APP) licensed in California to join our innovative remote healthcare team. In this work-from-home position, you will operate at the top of your license by conducting asynchronous patient evaluations. You will review comprehensive patient-submitted health questionnaires and utilize your advanced prescribing authority to recommend and prescribe targeted Over-The-Counter (OTC) therapies.

 Key Responsibilities 
 
 Asynchronous Clinical Evaluations: Review and assess detailed, patient-submitted health questionnaires to determine clinical needs, evaluate symptoms, and gather accurate medical histories. 
 Prescriptive Authority: Independently initiate, adjust, and write prescriptions for appropriate OTC products and therapies based on your asynchronous assessments, strictly adhering to California APh/APP regulations and established collaborative practice agreements. 
 Digital Patient Education: Provide clear, comprehensive written counseling and instructions to patients via secure messaging regarding the safe use of prescribed OTC medications, potential side effects, drug interactions, and lifestyle modifications. 
 Clinical Documentation: Maintain accurate, timely, and HIPAA-compliant documentation of all patient assessments, messaging, and care plans within the digital Electronic Health Record (EHR) system. 
 Triage &#38; Care Coordination: Identify red flags within patient questionnaires and appropriately refer patients to other healthcare providers, urgent care, or specialists when their medical needs exceed the scope of asynchronous OTC management.

 Competitive Pay: $85.00 to $95.00 per hour, commensurate with experience.

Essential Functions Review all medication safety events (Midas eQMMs) and participate in Root Cause Analysis to proactively identify opportunities to improve medication safety and reduce patient risk.  Position Summary: &#xa0; 
 The Cardiac Transplant and Advanced Heart Failure Clinical Pharmacy Specialist is responsible for the provision of comprehensive clinical pharmacy services for inpatients at the University of Rochester Medical Center.&#xa0; This individual serves as a resource for the interprofessional team and other pharmacists.&#xa0; Participation on multi-disciplinary rounds including pharmacokinetic and therapeutic consultation will be performed.&#xa0; The pharmacist will apply comprehensive medication management principles to optimize outcomes in a patient- and family-centered approach.&#xa0; Prepares, compounds, and dispenses prescriptions and medications for patients of SMH (pediatric, adult, geriatric) in accordance with all applicable state and federal laws and regulations for Pharmacy practice.&#xa0;

 Clinical Assistant Professor - School of Pharmacy 
 JOB PURPOSE:? ?&#xa0; 
 The University of Wyoming School of Pharmacy is seeking to hire one full-time (12-month appointment), non-tenure track faculty member with expertise in inpatient internal medicine services.&#xa0; This faculty position will be hired at the level of assistant professor within the Department of Pharmacy Practice.&#xa0;Individuals in this position will contribute to teaching, service, and scholarship, supporting the mission and vision of the University of Wyoming School of Pharmacy. Teaching responsibilities include experiential instruction at Banner North Colorado Medical Center, as well as didactic teaching on campus in team-taught longitudinal course series.&#xa0;Additional responsibilities include scholarship, academic service, professional &#38; community service, and other duties as assigned. 
 Faculty members are expected to develop internal medicine services at Banner North Colorado Medical Center in Greeley, CO (1.5 hour drive southeast of Laramie, WY), which has a long-standing relationship with the University of Wyoming School of Pharmacy. &#xa0;Within this inpatient role, the faculty member would be integrated as a member of the inpatient team associated with the North Colorado Family Medicine Residency training program. Faculty members would be expected to travel to the main campus in Laramie, WY, for didactic teaching and service activities that require a physical presence on campus.&#xa0;

 JOB SUMMARY

 As a member of a multidisciplinary health care team, the clinical pharmacist provides consistent clinical, consultative, research, and/or administrative pharmaceutical services for critical care, oncology/hematology, other advanced practice areas or participate as part of the rounding team for acute care areas.  Key Responsibilities: Inpatient Pharmacy Operations (Approx. 8 hours/week) 
 
 Provide inpatient clinical pharmacy services in accordance with departmental and hospital standards.

 Evaluate medication orders for appropriate drug therapy, dosing, and monitoring; identify and address potential adverse drug reactions.

 Work directly with frontline pharmacy staff to identify, troubleshoot, and resolve workflow, system, and technology?related issues. 
 Partner with the Inpatient Willow Team to assess Epic workflow or build issues and recommend optimal clinical solutions. 
 Participate in Epic Willow Inpatient testing, validation, and optimization activities.

 Provide support for planned and unplanned system downtimes, including participation in informatics on?call coverage.

 This is not an Epic Willow Inpatient Builder role. The position requires advanced Epic knowledge and close collaboration with Epic analysts and builders rather than primary ownership of Epic build. 
 Minimum Qualifications

 Primary Job Responsibilities:
