Position Overview
Leadership: Serves as a pharmacy team leader to service line(s) and other healthcare professionals. Embraces changes necessary to improve efficiency/effectiveness and acts as a change agent. Establishes collaborative with key stake holders to achieve optimal clinical and financial outcomes. Acts with honesty and integrity and exemplifies Novant Health's Cause. Participates in process improvement projects. Demonstrates leadership skills including problem solving, delegation, and time management.
Patient Care: Responsible for optimal medication management and promotion of medication safety. Participates in direct patient care activities and rounding opportunities where available and in alignment with department and organizational goals. Monitors therapy for appropriateness, dosage, effectiveness, interactions, and adverse drug reactions. Performs patient assessments. Reviews and acts upon clinical decision support surveillance. Completes organizational and/or departmental required documentation. Participates in patient education and wellness-based initiatives as driven by scope of practice. Prescribes and/or administers medications as allowed by law and scope of practice. Maintains up to date, evidence-based knowledge required for services provided. Reviews and processes orders specific to the population served. Supervises, checks, and controls quality and accuracy of work performed by pharmacy team members. Prioritizes workflow ensuring all services are performed in a timely manner. Maintains and follows formulary approvals, interchanges, stocked medications and non-formulary process.

Ensure timely responses to service line requests and inquiries
Quality Improvement: Demonstrates knowledge of process improvement principles and participates in continuous quality improvement projects and promotes medication safety. Pursues and maintains knowledge required for services provided. Participates in development, review, and validation of order sets, protocols, policies, and plans of care. Serves as a contributing member for service line and pharmacy committees by leading and supporting initiatives. Responsible for reporting metrics to leadership team. Participates in annual formulary review. Proactively identify other cost savings or appropriateness of therapy initiatives. Supports and guides facility-based or system wide medication use evaluations and participate on MUE project teams/workgroups. Develops timely system-wide plans for drug shortages and implementation with communication to pharmacy department, management team, procurement team, as well as nursing and physician partners. Develops, designs, and implements service line outcomes projects and clinical initiatives as directed by leadership team.
Team Development and Education: Develops and delivers effective education to pharmacists and other healthcare providers. Serves as a preceptor to residents and students. Participates in the onboarding of pharmacists and other team members. Provides ongoing feedback to fellow pharmacists to achieve desired outcomes. Serves as a mentor to pharmacy team. Monitors and enhances compliance with clinical programs. Participates in revision of clinical CBLs and development of annual clinical competencies.
Compliance and Organizational Alignment: Supports Novant Health policies, system-wide and department initiatives, regulatory and accreditation standards, and quality/process improvement efforts. Successfully completes, in required timeframes, annual competency requirements to support training and education, accreditation, and licensure. Meets P&T deadlines and presents P&T items as assigned and following detailed P&T process and templates.
Meetings and committees: Attends all pre-P & T meetings, P & T meetings and post P & T meetings if presenting. Attend and participate in MUE project team/workgroup meetings as assigned. Attends residency team meetings as applicable. Attends service line/institute best practice teams, councils, and accreditation teams as assigned. Attends pharmacy department meetings as scheduled.
Critical Care and Cardiology Clinical Pharmacists work a full-time schedule of five 8-hour shifts and are required to work a weekend rotation (every 3rd weekend).
Requirements
Current North Carolina Board of Pharmacy Licensure, or must be completed within 30 days of start date by either examination or reciprocity
Board of Pharmacy Specialties Certification or other approved certification must be obtained within 2 years of start dates and continuously maintained while employed
PGY1 or 2 years of experience as a registered pharmacist in health system pharmacy; PGY2 preferred
Critical care or Cardiology experience required (Residency will suffice in lieu of specified experience)
