Position Summary: The Cone Health Specialty Pharmacy team has openings for a full-time (1.0 FTE) Clinical Pharmacist III at the Cone Health Rheumatology Clinic and our Pharmacotherapy Clinic.

The Rheumatology Clinic Pharmacist will practice in an ambulatory setting providing specialty pharmacy and clinical services to our patients.  We are seeking an individual who is interested in obtaining a Clinical Pharmacist Practitioner (CPP) license and developing a collaborative practice agreement with our providers, precept students/residents, and help advance our ambulatory and specialty pharmacy practice models.  Hours are Monday - Friday, 8:00 am - 5:00 pm with remote work/administrative day opportunities.  Specialized experience and/or a residency with strong ambulatory experience is preferred.

Cone Health is seeking highly motivated clinical Pharmacist(s) to fill openings in our specialty pharmacy service line. Successful candidates should have advanced knowledge and expertise to quickly assess clinical data and deliver patient care across a variety of disease states. The roles will be hybrid and based in the Specialty Pharmacy Pharmacotherapy Clinic. Pharmacists will oversee medication therapy, make clinical decisions and recommendations, and follow up clinically with patients throughout treatment. Candidates should be familiar with specialty pharmacy operations and disease states such as neurology, infectious diseases, inflammatory diseases, gastroenterology, cardiology, oncology, and others.  Scheduled hours are hybrid between 7:30 AM – 5:30 PM and include an on-call rotation. 

RESPONSIBILITIES:

• Interprets and follows physician instructions to deliver the correct care to the correct patient.
• Provides and monitors drug utilization and medication-related information to healthcare professionals, patients, and the public.
• Supervises and evaluates pharmacy technicians.
• Precepts students and residents.
• Functions at a higher knowledge base and competency and spends most time addressing complex situations or works on project improvement initiatives.
• Performs other duties as assigned.

Required or desired credentials or experience of applicants:

Required Degree(s): Pharm.D.

Experience Desired:  Specialized experience and/or a residency with strong ambulatory experience

Desired Certification(s): Board certification in a specialty area or willing to get board certification upon hire
