Join our team at Ascension Saint Thomas Medical Partners as an Ambulatory Care Pharmacist Specialist at our offices in McMinnville, TN. Collaborate with a dedicated team under a progressive collaborative practice agreement to manage patients with chronic conditions such as diabetes, hypertension, asthma, COPD, and more. This pharmacist will:
Provide face-to-face comprehensive medication management services to rurally located patients in McMinnville, TN
Offer virtual CMM services to other rural practices in the Middle TN region including Centerville and Sparta.
Establish a hospital-based Pharmacotherapy Clinic inspired by our sister ministry which won a 2023 ASHP Best Practice Award.

What We're Looking For:
PharmD with active Tennessee pharmacy license.
Previous experience practicing under a collaborative practice agreement.
Preferred: PGY-2 Ambulatory Care training or equivalent experience.
Strong clinical knowledge in chronic disease management.
Excellent communication and patient counseling skills.
