PGY2 - Ambulatory Care - Milwaukee WI - The PGY2 Ambulatory Care - Aurora Health Care Metro, Inc. residency program located at Aurora St. Luke’s Medical Center in Milwaukee, WI provides opportunities to integrate into a variety of ambulatory clinics, virtual population health, and pharmacy transitions of care services. This program prepares residents to specialize in advanced ambulatory care specialties through interprofessional collaboration to enhance access to patient care. Residents will provide direct patient care within interdisciplinary team settings, practice under collaborative practice agreements with the opportunity to enroll and bill for services through pharmacist provider status within select clinic locations. Residents actively collaborate, build strong relationships and coordinate patient care between ambulatory clinics, outpatient pharmacies, and other hospital departments. PGY2 – Pharmacy Informatics - Milwaukee WI – Based out of Aurora St. Luke’s Medical Center (ASLMC), the PGY2 Pharmacy Informatics Residency supports medication use as an enterprise for Advocate Health Care. Advocate Health Care is a substantial health system, spanning six states in the Midwest and Southeast and contains 69 hospitals. The program provides specialized training in all aspects of pharmacy informatics and automation as part of the pharmacy information systems team. Requirements:
Registered pharmacist in the state of Illinois or Wisconsin
Bachelor of Science or Doctor of Pharmacy degree from an ACPE accredited School of Pharmacy
Completion of an ASHP-accredited PGY1 Pharmacy Residency
