Under the direction of the Clinical Pharmacy Manager, The Clinical Pharmacy Specialist- Internal Medicine is responsible for working directly with the patient care team to provide comprehensive pharmaceutical care for the patients of the assigned service area(s). The incumbent is responsible for the optimization of drug therapy, control/analysis of the formulary, and the identification of drug related problems. He/she will serve on organizational committees and be an ambassador for best medication practices.
Participation in rounds with multiple medical specialties individually and in a group setting.
Create and foster a clinical pharmacy practice in the department.
Coach and mentor fellow staff pharmacists, residents and students
Provide clinical consults to physicians and medication therapy management to high risk patients
Assist with medication adherence and optimal use of medications for commonly treated conditions
Drug utilization review and analysis.
Superior communication skills, including developing and giving presentations.
Knowledge of clinical best practices and standards for optimizing medication use
Optimize patient outcomes by ensuring appropriate medication use and drug affordability.
Work with other members of the PHO/ACO teams in conjunction with providers to optimize individual patient outcomes
Respond to specific requests for medication information from health care providers
Review primary literature and guidelines to create and implement department best practices.
Prepare and present educational materials
Proficiency in Microsoft Office applications, including Outlook, Excel, Powerpoint and Word
Perform other similar and related duties as required or directed.
Required: Current MA Pharmacy License or MA license eligible.
Required: Graduation from an ACPE-accredited school of pharmacy with a PharmD or equivalent.
Required: PGY 1 Pharmacy Residency training OR at least 3 years of experience in a role such as or similar to: Clinical Pharmacy Specialist or Clinical Pharmacy Coordinator.
Preferred: Board Certification OR PGY 2 Pharmacy Resident Training
