Manning Family Children's is currently seeking Clinical Pharmacist candidates for expansion of services to our Hematology/Oncology teams. As a member of the Pharmacy department, the Clinical Pharmacist will provide pharmacy services to their respective area and work as part of a multidisciplinary team to deliver safe, appropriate, comprehensive, and cost-effective medication therapy.

Job responsibilities include the following:

• Participate in multidisciplinary rounds with the medical team, as applicable
• Review drug therapies and apply appropriate pharmacokinetic and pharmacodynamic principles to provide optimal drug therapy recommendations
• Interview and counsel patients and/or families regarding safe and effective medication use
• Develop and implement clinical practice guidelines to enhance pharmaceutical care and medication safety
• Participate in medication use evaluations and formulary management
• Serve as a drug information resource for providers, nurses, fellow pharmacists, and other healthcare professionals
• Detect and document adverse drug reactions, clinical interventions, and medication errors
• Serve as a preceptor for the PGY1 Pharmacy Residency Program and Residency Track Advanced Pharmacy Practice Experience (RT-APPE) program

Requirements:

• Doctor of Pharmacy from an accredited college of pharmacy
• Current licensure or eligible for licensure by the Louisiana Board of Pharmacy
• Completion of a PGY1 Pharmacy Residency AND
• Completion of a PGY2 Pharmacy Residency OR PGY1 Pharmacy Residency with hospital pharmacist practice experience (2-4 years preferred)
