The Hoag Clinical Pharmacist - Clinic is a licensed pharmacist who is responsible for applying knowledge, skill, competence, and clinical expertise in the outpatient Hoag Clinic. Working with a multidisciplinary team, the Clinical Pharmacist will primarily work in the Hoag Clinic as a direct patient care clinician. The Clinical Pharmacist - Clinic will work closely with our providers to add, modify, or discontinue medications and order and interpret laboratory assessments as needed for consulted conditions per protocol. The pharmacist will see patients referred by physicians and providers in an integrated care environment, contribute to training and education of other clinicians, and participate in care management, utilization management, and quality improvement initiatives. In addition, the pharmacist will provide medication therapy management including examination of patients’ medical records, performing a comprehensive medication review, analyzing medication lists for potential patient safety problems (including drug interactions, suboptimal medication regimen, dose adjustment, etc.) and applying evidence-based medicine and national guidelines when optimizing the patients’ medication regimen.
Additionally, this position will co-manage high-risk patients with PCPs through in-depth disease education, home blood pressure monitoring review, diabetes coaching and insulin titration protocols, and addressing medication barriers.
The Clinical Pharmacist - Clinic displays and practices a work-style that reflects the mission, vision, and values of Hoag. Required Education and Experience
Graduated from an ACPE-accredited pharmacy school program is
At least 3+ years experience in ambulatory care setting or related services in a progressive healthcare system, OR completion of a post-graduate year one ASHP-accredited residency program +/- post-graduate year two related to specialty training program
Experience in a Medical Group, Health Plan, and/or Risk Based MSO setting.
Experience with value-based programs, including but not limited to Medication Adherence, Statin Adherence, Poly-Pharmacy initiative, and other HEDIS/PQA metrics.
Have an understanding of nationally recognized criteria Milliman Care Guidelines/Interqual/CMS, etc. preferred.
