We are recruiting two Clinical Pharmacy Practitioner/Ambulatory Care Pharmacy Specialists at WVU Medicine - WVU Hospitals!
1. Rockefeller Neuroscience Institute at is seeking an Ambulatory Care Clinical Pharmacy Practitioner to support our specialty neurology clinics with an emphasis on memory and movement. This is an exciting opportunity to contribute to a renowned academic neuroscience program while shaping the role of pharmacy within a specialty ambulatory setting.
2. The Medical Group Practice has an established group of Ambulatory Care Clinical Pharmacy Practitioners that work in primary care/internal medicine clinics managing disease states including but not limited to diabetes, hypertension, hyperlipidemia, anticoagulation, and weight management.
In both roles, the pharmacy specialist will work collaboratively alongside clinic providers to optimize medication management, support patient care, and enhance clinic workflows. Our positions offer the opportunity to establish a presence in the clinic through a collaborative practice agreement, providing direct patient care, medication education, and chronic disease management. In addition to patient care, these specialists will help precept and mentor student and resident learners.
Must possess current license as required by state board where services will be provided: WV: Pharmacist license through the West Virginia State Board of Pharmacy
Obtain certification in Basic Life Support within 60 days of hire date.
