Clinical Pharmacist Specialist (Multiple Openings: Medical Oncology, Solid Organ Transplant)
Job Description: Provides an expert consultation, guidance, and direction for appropriate medication use. In conjunction with a team of physicians, provides complete care to the patient through consultative services and active participation in multidisciplinary patient care planning meetings. Optimizes the outcomes of patients by providing evidence-based, patient-centered drug therapies and monitors for drug-related problems and medication cost containment. Supports regulatory requirements and local quality programs related to safe medication use and for systems to promote medication stewardship and surveillance. Provides pharmacokinetic and/or pharmacogenomic consultations, when applicable.
